What does --update do?

I looked at Handbook:AMD64/Working/Portage#Updating_the_system, this page, Upgrading Gentoo and emerge(1), but only the man page has an explanation:

Updates packages to the best version available, which may not always be the highest version number due to masking for testing and development. Package atoms specified on the command line are greedy, meaning that unspecific atoms may match multiple versions of slotted packages. This option also implies the --selective option.

But sometimes packages update even when this option isn't supplied. For example:

 * An update to portage is available. It is _highly_ recommended
 * that you update portage now, before any other packages are updated.

 * To update portage, run 'emerge --oneshot sys-apps/portage' now.
